I have a party/event based website and need to take reservation (Name/Email/etc. etc.)

Basically, I need this page,http://www.asianfriendsparty.com/modules/tinycontent/index.php?id=12
to work.

Is there a existing module out there? I couldn't find it.

You might try Event Guide. It may be "more" than what you need but it should be able to be adapted relatively easily.

Here's a description from the authors website.

Event guide module will display event guidance and reservations via forms.

The modules have functions as follows:

- Event reservations
- Configurable reservation forms
- Display event automatical in time
- Adminstration reservations and get data CSV format
- Mail to send reserved person
- New event notify to registers

I believe this module will work fine.

EDIT: thanks... fixed the link. (it was the same as the one under my signature.)

Login to the upnrunnin site, using demo2 ... feel free to create a new forum or edit the test form. I would like a test template... along the lines of what I was inquiring about on the Dokuwiki page. I plan to concentrate on learning Formulize and Dokuwiki. (and the programing language that goes with them)

To many different modules is information overload for me, so I dropped CBB and Polls.

I would truly appreciate yours (& others) input!!!

Yes - Formulize will give you the functionality you need to create the form you showed.
