Can't register new user?

my wife just tried to register for my site and she gets an error message

"Could not register new user"

I had a user register just last night - what could be causing this error?

I'm using v2.0.13

I tried to register and it worked ok. Are you sharing an email address or password?

I was able to register her from my machine but it didn't work from hers - I'll have to see if she was using my email address instead of hers, I think she was using her own, though.
