Pb resolved :
In administration groups, The module don't appear for "Acces rights module"

So :
1) In /formulaire/xoops_version.php
$modversion['hasMain'] = 0;

become
$modversion['hasMain'] = [color=FF0000]1[/color];


2) update module formulaire
3) set to 0 in module (invisible in menu)
4) Enabled "Acces rights module" for yours groups

That's all !!!!!!!!!!!!!
