Is there any possibility that the image manager automatically creates a new category for each user, that can only be accessed by the user?
What I mean is if a user uploads a pic with the image manager I don't want that other users can access or use the same picture. Is there any way to change it or any module to replace the image manager?

There are a few photo gallery modules that allow people to have personal photo albums...I think extgal (new one, quite nice) and xcgal, but probably others too.

The built in image manager works on group-based permissions only, so it probably wouldn't be practical to use it for what you want to do.
