I have been searching for a module or some code snipit to add one or more file upload fields to a form so users can submit files with text in a form.

Any ideas? I have not found anything that seems viable.

Thanks! I installed the latest version of WF-Downloads and the WF-Dowloads patch form the link you provded and I do not see any upload options. I enabled it in the Prefrences but not sure what else to do. I will probally NOT have any downloads to offer but I want people to submit images with content. I have an some open issues with Formulize so I am not sure that will be a viable option for me. Any suggestions for using WF-Downloads for uploading?

I understand technobia has elected to use Liase, but for anyone coming across this thread looking for info...

WF-Downloads is a file distribution module. You create categories in it and upload files to the categories so other people can download them. You can also simply post links to files hosted elsewhere.

The integration patch for WF-Downloads that links it up with Formulize simply lets you create custom forms for each category in WF-Downloads. This can be very useful since the default form that WF-Downloads uses for all categories is heavily software-centric. If you are using WF-Downloads to distribute meeting minutes, curriculum files, promotional media, etc, pretty much anything except software, then a custom form is very welcome.
