A full description of the issue:

Note: I am installing XOOPS on a server that already has a pre-existing functional copy running a different domain.

I extracted the XOOPS archive and granted read/write access to the following files/directories...
/cache
/templates_c
/uploads
mainfile.php
Installation went off without a hitch but as soon as I clicked the link to view the site, I received a blank page. naturally, I turned on debug in MySQL and received the following errors...Any thought/ideas?

Make sure your template_c and cache folder have permissions set for the built in IUSR account.

If you read my post again you'll see that I've already given read/write access to the IUSR_machinename.

Just for kicks, I gave the IUSR "full control" permissions on both folders mentioned and received the following errors...

NOTE: The IUSR_machine name has full read/execute permissions on the entire XOOPS directory for testing purposes.

Then it looks like possible file corruption in transfer, it is complaining about not being able to read files. Did you upload via FTP? Make sure you are using binary mode for PHP file transfer. Just reupload the files should be sufficient to see if the fatal errors go away.

If this is a local machine, no idea of the problem, still could be corruption or permissions, I know if you select the file and choose properties, go to advanced you can get a cumulative assessment of permissions, that may help pin down the problem also. WS2003 sometimes touchy on permissions that may have been denied somewhere else will trickle down to where you need them.

I am not real up to date on Windows Server, but trying to help.

Due to the issue I've presented, I setup a virtual server...installed windows server 2003 standard edition, install all updates, installed iis6, installed php 4.4.1, MySQL 4.1.15 and XOOPS 2.0.13.2 (fresh download) no permissions set except allow *everyone full control* and the error persists... o_O

Try IIS-Resources for some other hints.
