CONTACT US form with user-selectable recipient

I have had a couple of clients interested in modifying the standard "Contact Us" form so that the user can select a specific recipient. For instance, the company might have several departments ("Sales", "Advertising", "Human Resources", "Product Support") with different people responsible for each department ("Sam", "Alice", "Henry", and "Paul"). A user could go to the website, select the name of the department they wish to contact ("Advertising"), type the message, and then be confident that the proper recipient ("Alice") would get the email directly.

Has anyone seen a "Contact Us" module for XOOPS that does this?

It could, but I have not personally tried it. I really got hooked on this mosule. Formulaire. But I do use the older version of laise also, but only to my PM address.

I tried using Formulaire a while back, but the docs were all in French and I couldn't get it to run properly. If there's a new version, I might give it a go.
