myalbum-p can't upload photos

A few months back my website users stopped being able to upload photos. Finally getting around to it I've tried updating to myalbum-p 2.84, no luck. I've tried updating XOOPS to 2.0.13.1 and then to 2.0.13.2, no luck. PHP Debug mode shows nothing.

When you enter the photo information and press either Preview or Submit the screen returns to a blank form. The same thing happens logged in as the administrator. I've double-checked the upload permissions, folder permissions, etc and haven't found the key yet.

Nothing showing up in the httpd logs, system logs, or PHP error logs.

Xoops 2.0.13.2
PHP 4.2.2
mySQL 3.23.56
Apache 2.0.40 (Red Hat Linux)
Browers: IE6, Firefox 1.0.4 & 1.5

After a clean install of 2.0.13.2 (and later 2.0.13.1 just to be sure), I get the main theme page (I've tried all three with the distro) with nothing except the top and bottom banners. No access to admin.php and user.php shows the same thing. I finally turned on Smarty debugging directly in the database and see the following:

{$xoops_showcblock} 0
{$xoops_showlblock} 0
{$xoops_showrblock} 0

Is there someplace I can force the display of the blocks so I can login and get to work?

PHP 4.2.2
MySQL 3.23.56
Apache 2.0.40 (Red Hat Linux)
Browser IE 6.0, Firefox 1.04, Firefox 1.5
