What did you do to the system before it happen?

Blocks cannot disappear out of nothing, unless there is change in XOOPS. I don't think that even upgrade in PHP by your host provider would cause that.

Are you sure that you have selected the right options in the pull-down menus?

The best way is to set all of them to "All types"

If you didn't do anything, your host must have changed something... Have you asked them if they've made any changes recently?
