Never saw this before today. I was creating a new form and got this error.

Formulaire is not using correctly the error reporting functions.
You have to turn debug mode on, to see more information about the (probably some kind of database) error.

I do have debug on and didn't see anything.
It may have been a duplicate form error but I'm not certain. Thanks for the response.

What version are you using? (and it's Formulaire, right? (see title)).
If you are cloning the form, there is a patch on frxoops.org .
