Are you using HTML for your banners or are you just pasting the click URL and the Image url in the appropriate fields?
Did you turn banners on in System Admin >> General Preferences >> Activate Banner Ads ?
Looking at your code, there is nothing being generated where the banners are suppose to be. It's like they are turned off.
