First things first. Get up and running with the standard XOOPS installation, then try accessing as admin and user.

When you get to that point, you can make a backup of your install and db to revert to in case you get into trouble, then move forward to the questionnaire. Profile does not support this to my knowledge, so you are probably looking at a module such as quiz, or a custom retrofit.

I have not used it but you can email an admin when someone registers and require admin approval.

Go to the admin menu and choose system options > user info settings

Select "Requires activation by administrators" under "Select activation type of newly registered users".

This will allow a user to register but they won't be activated until the admin activates them. This however doesn't verify the email is a correct one.

Under the option "Select group to which activation email will be sent" you will select the group you want to send the email to. All members that are allowed to activate users then need to be added to this group to receive the notifications.

Then once you have done that you need to add the fields you want to profile which is easy to do. The admin that is activating them then will need to look at their profile entry and look for the answers there.

There is no easy way at this time to automate this process further but you CAN do it without needing to do any recoding. It would be a royal pain if you have many new users at a time.
