We have a bug in our pedigree database and wonder if anyone can help me fix it please

We have an 'Add a Dog' feature and 99% of the time this works fine, but occasionally (usually if more than one person is entering data at the same time) we get a garbled dog record which is totally incorrect and contains odd characters.

My only workaround is to delete the dog and re-enter it. The original programmer did write a add on 'Optimise database' and when the problem pops up i run this and then we are ok for a while

I would like to elimiate this bug altogether those and would appreciate any help please
