Modules Medication Record

hello...

i have some problems.
first
i want to build modules to view medication record like image below

this record only:
1. views by own members record and admin
2. can edit by admin only

second
it is members can put in different group that group monitored by different admin
chart like this:

anyone can help me how to create this modules?

I know there are a few "how to build a module" guides and sample codes out there. I suggest you use the search feature on Xoops.org to find them.
