there's missing <{$xoops_module_header}> in your custom theme.
edit your theme.html and put <{$xoops_module_header}> just before javascript:
[color=ff0000][b]
<{$xoops_module_header}>[/b][/color]
<script type="text/javascript">
script>
save the file. and go to GENERAL SETTINGS and check "Yes" where it says "Update module template .html files from themes/your theme/templates directory?"
Now return to your home page and log out. It should work. Change Update modeule template etc back to "No."
