Can't Change Passwords

Users/Administrators can login and take the steps to change their passwords. The confirmation page says "database updated" and it looks like everything is good. But... alas, the password remains unchanged. If you autogen a new password using the lost password feature, the new password does stick. However, passwords don't seem to change otherwise. Please help me out so that users can change their passwords! Thanks!
