Hi,

I want to edit the post comment form to add there some custom lines for one module only. The other modules' comment form should stay wihout any changes. Where should I start from?

You can start by include/commment_form.php

but i believe that's gonna influence all the comment system thruout the website. I want it to be changed for the single module only.

Yes, just do a check on the module name or id.
