there are more features in 2.3 than in 2.0.18.2, plz check the announce that lists new features.
2.3 = 2.0.18.2 + new features
new features = 2 additional core modules (PM/Ext profile), CAPTCHA, improvements in blocks management, a new dhtml editor, etc. + mainly a preparation for 3.0
it depends if you need those new features, but you will have to upgrade to 2.3x soon or later, since only this branch and the 3.0 will be maintained starting from now. It's better to start with 2.3 if you are happy with it
