Problem with module : friendfinder_V3.3

and

PHP Version 4.4.1-pl1
MySQL 5.0.15-nt
Apache
reg globs off

With this I cannot get the submit form to work - it returns the blank form again without anything being written to database.

This module is excellent but sadly can't make it work - there are several websites listed for this module in this module's install.txt - not sure which one to go for such help.
