admin_forums.php Error with XoopsForge???

When I run admin_forums.php in one of my installations (Add newbb Forum function)I get the following error:

Notice [PHP]: Undefined variable: xoops_admin_menu_js in file include/cp_functions.php line 72

I am unable to add Forums unless I manually edit the DB. Any ideas what I need to do to fix this?

The screen also looks bizarre Resized Image

btw. this is my only installation using XoopsForge might there be a connection?

I am also having this issue on 2 of our sites. We have reloaded each XOOPS 2.6 completely and it works for a day and than we get the above screen. All else is working fine but this damn forum admin bug. Any help would be great.

all other req are meet.
