Here is what I did to fix the problem.
I looked at the core 2.3 Register.php file, and then opened the register.php and forms.php found in the profile module, and made the following updates:
modules/profile/register.php
line 126-130
xoops_load("captcha");
$xoopsCaptcha = XoopsCaptcha::getInstance();
if (! $xoopsCaptcha->verify() ) {
$stop .= $xoopsCaptcha->getMessage();
}
modules/profile/include/forms.php
lines 337-340

xoops_load("XoopsFormCaptcha");
$reg_form->addElement(new XoopsFormCaptcha());
}
And this resolved the problem.
