I have a site that uses a static front end with a users area run by xoops. Now the owner wants to have a custom subdomain for each registered user. For example:
http://www.mysite.com becomes xoopsuser.mysite.com
Xoops runs in http://www.mysite.com/users

Any ideas on how to achieve this?

I haven't done this ever and I can't (no mod_rewrite) but you should be able to do this with fancy urls, probably there is a lot of information around here, just go and search... this might be hard though!
I used to use this account, but no longer.
