Content module - adding a second top-level menu item

I have used the Content module to successfully set up a menu item and sub-item. Now I am trying to set up a second top-level menu item.

I created another document but a link for it is not appearing in the main menu. The content page looks like this:

The above configuration should be making:
A Main Menu Item for About Us
With a Sub-munu for History of Wonderfish
And a second top-level menu item for Test Cam

Shouldn't it?

The first entry works fine but the second main menu entry (Test Cam)will not show up.

So I thought OK I will simply install a second instance of Content Module - but it won't allow this.

What have I incorrectly configured? I have searched endlessly prior to posting this request.

Test Cam: submenu = Yes?
